New Scout Forms

When joining Troop 25, several documents are required to complete New Scout registration.   The links for many of these documents can be found below.

Please Note:  To camp with the troop, adults must be registered.  Please complete New Adult Paperwork in order to become a registered adult.

Submit all paperwork and registration fees to the troop secretary or treasurer.

BSA Medical Forms Parts A & B (3 copies) – must be completed electronically

Health Insurance Card – front & back (3 copies)

Immunization Records – if not completed on Medical Forms; can be obtained from doctor or school
